For your convenience we accept a variety of insurance plans, including may HMOs
(Health Maintenance Organizations), PPOs (Preferred Provider Organizations), POS
(Point of Service), traditional insurance, and Medicare.
Many of these insurance companies have their
own Web sites. If you would like to see if your insurance company has a Web site
and it is listed, simply click the button below.
When you arrive at our office for an appointment, with the doctor or nurse,
please provide our Front Desk Staff with your insurance card so they may make a
copy of it. The Front Desk Staff may also ask you for proof of
identification. Please do not be offended if we ask for this
identification as we may not be personally acquainted with you.
Also, if there are any changes in your insurance please let the Front Desk Staff
of the Family Medical Center of LaGrange know as soon as possible so we may
update our records. If you have changes, or a new patient, and you would
like to mail us this new information, simply click the link below and we will provide the form for you to print for standard mailing.
