Referral & Authorization Information

Referral Information:
At times you may need to receive care outside of our office, like X-Rays or
specialized consultations. Your insurance company may have specified a
network of physicians and facilities to offer this care. If you insurance
company has such a network, you must stay within it to get your claims paid.
To receive these services, most HMO and POS patients require and
authorization number or written referral.
Our referral staff has over ten years of experience in helping patients receive
the care they need. We know that this can be a confusing process and we
will be happy to assist you.
Please note that you need to talk to our Referral Staff BEFORE
you seeing the consulted specialist or getting X-rays, MRIs, etc.
To Obtain a Referral:
Schedule your appointment with the specialist, lab testing center, or
hospital.
Call our Referral Department Staff with the date on which you plan to
receive these services. Please give us 72 hours to provide your
request.
